INFORMED CONSENT FOR USE OF PLAY EQUIPMENT

By agreeing to the terms of service, one acknowledges that their child/children will be involved in play activities which may involve the use of specialized equipment, such as suspended equipment and various swings, large therapy balls, tactile or touch media, a climbing loft, a climbing wall, or a scooter board and ramp. Additionally, they understand that there is an inherent risk of physical injury with participation and accept that risk.
